Printed forms are available to businesses reporting nine or fewer properties. The Unclaimed Property Program requests holders reporting 10 or more properties to report electronically.

The forms listed below are of interest only to holders of unclaimed property: banks, businesses, corporations, financial institutions and other entities required by Pennsylvania's escheat laws to report their holdings of unclaimed property. Reports are due by April 15 of each year, and property must be sent to the state for safekeeping.
