Alaska Unclaimed Property Forms and Instructions

Reports with 20 or more properties must be processed with electronic reporting software and submitted on a disc or CD. Magnetic tapes are not accepted.

There are two sections to the Reporting Form. The Verification of Unclaimed Property Form (04-720A) and the Report of Unclaimed Property Form (04-720B). When filing the Reporting Form, please complete both forms.
