Holder Information (Florida Unclaimed Property)

Florida unclaimed property holders must create a DFS-UP-111 Cover Sheet for "online filing." To do this, they need a Holder Number:
If you do not know your holder number, contact the Holder Reporting Section at (850)413-5522. If you have not been assigned a State of Florida holder number, you need to request one.
To request a State of Florida holder number, please provide the following information by mail or fax:
Holder name, federal employer's tax id number, address, city, state, zip code, contact person name, contact person telephone number, contact person fax number, contact person e-mail address and the standard industrial code number (SIC code list is located on the BUP website under the Reporting Instructions-Holder Reporting Links and Information section).
